Opportunity Village offers outstanding variety in services, to meet the individual needs of every person served. The one constant is "person-centered planning." This means we work with the person served and his or her family to find out their hopes and dreams, their needs and goals. As a team, Village staff helps the individual set a course for success. If you are applying for services, you will also need to download, print and complete the Social History Form. It should be mailed with the Application for Services to Elisha Payton, as above.
