The Village actively seeks candidates for admission to all levels of service.  Generally, this needs to be coordinated with your county of origin to procure funding.

You may download and print the Application for Services or request them from Ms. Payton.  When completed, this form then may be mailed to Service Coordinator Elisha Payton, Opportunity Village, PO Box 622, Clear Lake, IA 50428.
